Source of issue –Drawings - LRQA Audit 16OCT15
Description –
LRQA Audit dated 16OCT15. Review of ICM job PO 20170, our quote J060715C. Previously supplied in A1 (according to drawing CD1741 r1), but for this PO, they requested A6 alloy. we supplied anodes to A6, Cert of Compliance 945-35521 refers to A6 alloy, but also drawing CD1741 r1, which was A1. The alloy composition was the only difference, all other details remained the same.
Immediate action –16OCT15: Re-issued certificates, "945-35521 A6 CC ICM rev2" and "945-35521 A6 CA ICM rev2". New drawing CD1781 rev1, was created for future orders in the A6 alloy.
Root cause –Drawing not aligned with Certificates
Corrective action –
To prevent re-occurrence, we will create a new drawing with the alternative alloy, this will ensure that the drawing and the Certificates will align in future. This requirement has been communicated to the 3 staff members involved with drawings, RN, JL and BL.
Verification –11DEC15: Random check found that all drawings and certificates had matching details. Also found evidence of new drawings being created for "minor" changes. This CAR will remain open for at last 6 months, when a more thorough audit will be conducted to verify compliance. If all is OK, CAR will be closed.
