Source of issue –Ian Moss from Whitsunday Diesel
Description –
Ian rang as he had recent issues with CDZ9-035 and CDZ9-071 (parted off from CDZ9-035) anodes. They were breaking off as they were being screwed in.
Immediate action –replace 10 of each anode types.
Root cause –New staff not filling the shot correctly + a number of maintenance/repair issues with diecasting machine.
Corrective action –
1. Increased training for new staff.
2. Ensure aging equipment maintenance is effectively carried out.
3. Weighing each casting
28AUG24: The casting process for this anode now includes weighing post casting. No incidents have been reported since JUL23, so this issue will be Closed.
