Thales – Delivery unsigned.

    Source of issue –Thales - O/N 3000076210/ Inv 64514 Delivered but not signed for
    Description –

    Davide Milite contacted CAA letting us know that an Aust Post delivery was left in a "secure place" but not signed for. Package went missing. In mean time SP arranged with Michael Lewis to send replacement anodes with TNT. SP subsequently explained this to Davide.
    In Davide's words....
    1. Future Deliveries: Please ensure all future orders are dispatched via a reliable courier service that provides tracking, signature on delivery, and delivery to the designated receiving dock (not left in "safe places").
    2. Carrier Instruction: Clear delivery instructions must be provided to the carrier, stating that deliveries must be signed for by a Thales staff member at the dock.
    3. Supplier Responsibility: As the supplier, we rely on your team to coordinate and ensure appropriate delivery methods are used, particularly when dealing with commercial and time-sensitive goods.


    Immediate action –SP sent replacement anodes.
    Root cause –Use of Aust Port rather than delivery service with tracking and signing on delivery
    Corrective action –

    1. Note in Thales MYOB file indicating not to use Aust Post.
    2. Note in DOD MYOB file.
    3. Note in Outlook.
    4. SP training with Dispatch Staff.
