Source of issue –Scope Engineering Received the incorrect anode type
Description –
11AUG17: Nick Wills from Scope Engineering placed O/N 6569 for 6 x CDZ8-18H anodes.
23AUG17: Nick emiled to say that they recieved CDZ8-18S anodes.
Immediate action –Scope needed the anodes urgently so we contacted Waterfront Marine (Lincoln Grose) who had them in stock in Fremantle. Scope went and collected them straight away. They were happy with solution.
Root cause –Stores: Did not read the PO correctly.
Corrective action –
Store person, Mitch Seidl made aware of error and management re-enforced importance of reading the requirements of all orders carefully.
We supplied the incorrect CDZ8-18S anodes to Yacht Grot, FIS.
Leave open for 6 months.
Verification –11MAY18: No similar incidents have occurred since this date.
