Source of issue –Customer: Richardson Devine
Description –
BL emailed quote B150823B, but provided rev1 or drawing CD1927. Rev 1 of the drawing has the anode 40mm thick. We provided a sample, which was 45mm thick. When they got the sample, Mick questioned the 5mm difference.
BL explained the difference as follows....When we actually cast the anode, we found that the bottom spigot (50mm major dia and 30mm minor dia) + the 32 x 3 insert, did not allow enough space above them for the Zinc to flow effectively. To overcome the issue we cast the anode 5mm thicker.
BL should have provided rev 4 of the drawing. Not rev 1.
Immediate action –Let Richardson Devine know we could cast either height, but would need to make new tooling if 40mm high. They chose to stick with standard 45mm high anode.
Root cause –BL incorrect drawing revision.
Corrective action –
See "Immediate Action" above.
BL updated all of the superseded drawings (rev 1, 2 and 3) to say SUPERSEDED in the file name. This should prevent this problem from re-occuring.
