CDZ4-100 Faulty Thread

    Date – July 8, 2020
Source of issue –Cullen Bay Slipway/Wheelhouse Marine
    Description –

Wendy Bateman emailed indicating that a CDZ4-100 could not be tightened. RG asked for photos, which indicated that the thread on the anode had been partially stripped.

Immediate action –Replace anode on next order, free of charge
Root cause –What has been occurring recently was the OD of the S/S shake proof washers vary in size, consequently some oversized washers have been difficult to seat at the bottom of the mounting hole, when the SHCS is screwed into the other half of the casting, the thread of SHCS has been damaging the 1st 3 threads of the tapped hole.
    Corrective action –

Our supplier (Suncoast Bolts) could not guarantee the supply of correct size washer so we decided to change over to a spring washer, hopefully problem solved.

Verification –As of 24AUG21, the issue has not re-occred.
