Source of issue –MG Kailis followed up with with status of PO
Description –
05JAN18: MG Kailis emailed PO KM042607_PO-1 to BL with email note from Marc Campbell requesting current prices and indicating that they would send amended PO.
08JAN18: BL emailed quote with current prices.
Never received amended PO, so PO not forwarded to JL for processing
Immediate action –Cast and dispatch anodes ASAP
Root cause –BL expected amended PO, but did not follow up when it did not arrive.
Corrective action –
1. All POs to be sent to sales@cathodicanodes.com.au where multiple people receive them.
2. BL (& all staff) to follow up when a customer does not email an amended PO.
