Anode Length in Costing Did Not Match Mould

    Date – January 16, 2017
    Source of issue –JL identifed error when preparing to cast anode
    Description –

    Anode Quote B150816B (rev 1, 2 and 3) was costed based on using Mould no. 1, which is 1508mm long, but the anode length was entered into the costing as 1600mm long (refer to drawing CD1820 rev1 and rev2).


    Immediate action –JL amended the drawing, re-issued to customer (CCE QLD) for approval (CD1820 rev 3). Customer (Dissie Kruger) was satisfied with revised drawing. Anodes cast to comply with revised cross section.
    Root cause –BL error while entering dimension into costing spreadsheet.
    Corrective action –

    To prevent re-occurrence, the costing spreadsheet was amended to ensure that only values up to the maximum length (and height) of the anode can be entered into the applicable fields (Data > Data Validation, allow Whole Numbers Between 1 and Mould Dimension Field for Length)

    Verification –Spreadsheet to be checked when next costing anode
