CDZ9-146 – Squashed Threads

    Source of issue –Supplier: IFS
    Description –

    1000pcs of CDZ9-146 machined against PO 21554 and DRG CD1306 r2. Inspection indicates majority if not 100% of stock damaged beyond use. We only just dipped into this stock so issue only just discovered.

    It appears that the machining set up utilised the collet gripping the male thread (pressure diecast anode parted to length) and has severely damaged the thread. Most cases the thread has been flattened, bent and torn.


    Immediate action –LS propose sending down CDZ9-033 to re-make with a change in machining methodology (chuck from opposite end) before remanufacturing.
    Root cause –
    Corrective action –

    1. Awaiting reply from IFS regarding rectification.
    2. CAA staff to ensure that goods are inspected upon receipt.
    28AUG24: All replaced by IFS. Their method of gripping the threads malfunctioned. We bore the cost of the replacement material. LS did not push them too hard on this issue as their price is significantly less than the next best. They rectified the malfunction and we have a great relationship with IFS.

    Verification –19AUG25: No similar occurrences over past 12 months.
