Source of issue –Customer: The Boat Warehouse
Description –
New customer, TBW rang to say that they ordered CDZ8-123-150H (PO 12299) and were invoiced for CDZ8-123-150H (inv 61418), but customer in Broome WA received CDZ8-123-150S.
Immediate action –CAA sent replacement anodes directly (in unmarked packaging) to TBW customer in Broome.
Root cause –Stores staff sent incorrect anodes
Corrective action –
Stores Staff provided with additional training to ensure that they pay closer attention to ensure that they dispatch the correct anodes. Pay particular attention when product codes are similar.
Verification –No further incidents recorded
