Source of issue –Allen Chalmers
Description –
As part of order/invoice 49775, we supplied 30 x CDA-1.55S Type 1 anodes to Chalmers Marine with the standard 350mm long straps. They were supposed to be custom straps, 410mm long.
Immediate action –50% discount on the 30 anodes that were supplied, provided as a credit.
Root cause –The custom straps were noted on the invoice/PO, but not highlighted.
Corrective action –
In future, custom anodes or any requirements that are out of the ordinary are to be highlighted (BL to do this on the invoice/PO on the PDF). Also, BL to include any non-standard requirements in the body of the email accompanying the PO.
At the instigation of LS, all staff involved with sales were emailed a re-iteration of the process with some improvements (as above) as a refresher on the process to be followed to ensure that information regarding non-standard anodes is accurately conveyed..
Verification –Has similar issues occurred? Leave open for 6 months. As of 24AUG21. No further issues.
