Source of issue –Customer - Incat Trasmania
Description –
We supplied 40 x CDZ8-126-200H anodes to Incat in December 2019. On the 15th May 2020, David Cross from Incat informed BL that they were the incorrect anodes. We supplied the standard CDA8-126-200H, rather than the CDA8-126-200H TYPE 1. It is the same anode mould, but the Type 1 has an insert with 13dia x 31 elongated holes.
Immediate action –Incat could not use or modify, so we replaced.
Root cause –It was 5 months ago, but we believe the TYPE 1 was not written on the board
Corrective action –
This issue to be highlighted at the Toolbox meeting to ensure the correct information (inc drawing no.) is included on the board. General awareness of ensuring that order details are verified during casting and at dispatch. Part numbers can be confusing, so staff need to be aware that the correct anodes are cast.
